MODEL RELEASE FORM
I hereby irrevocably consent to and authorize the Bureau/Foundation or those authorized by the Bureau/Foundation the right to use, prepare, and reproduce images that have been taken of myself in any digital, video, photographic and/or other audio/visual formats, and thereafter distribute, publicly display, and publicly perform for commercial or educational purposes in any medium or communication now known or later developed, including, but not limited to: websites, social media, conferences and seminars, magazine or newsletter productions, videos, CD-ROMs, event brochures, and/or other collateral marketing materials related to this event without compensation to me.
I hereby waive any right that I may have to inspect or approve the finished work(s) or the use to which it may be applied. I agree that the recordings taken of myself in any format(s), and any resulting reproductions shall constitute the Bureau/Foundation's sole property, to copyright in its own name and with full right of disposition in any matter whatsoever.
Similarly, I hereby grant and assign all rights, title and interest which I may have in and to such photographs to the Bureau/ Foundation, and release and discharge the Bureau/Foundation from any and all claims, demands or liabilities arising out of or in connection with the use of the photographs, including any and all claims for libel or invasion of privacy. I also release and forever discharge the Bureau/Foundation and their agents, licensees, heirs and assignees from any and all claims that I may have that relate in any way to the use of the publication of my photograph.
